Is the Quality of Service better with Higher Competition among Long-Term Care Institutions?

Abstract

|
The purpose of this paper is to empirically analyze the relationship between the level of competition between service providers and the quality of services, especially of home-visit care services. The data used here for analysis data are the National Health Insurance Service¡¯s data on service provider performance and reimbursement claims. The authors calculated Herfindahl-Hirschman indices by the ratio of expenditure of home care expenses in the region; the service quality of the home-visit care providers was measured using the results of evaluation of long-term care institutions. As a result, the level of service supply competition in the region did not have a significant positive effect on the service quality of the home care institution, and the change in the level of competition did not induce a change to improve the service quality level at a significant level. This study confirmed that the policy goal of pulling up service quality improvement through service supply competition has not been achieved effectively. However, provider¡¯s size and status as a corporate entity had a positive relationship with service quality. Therefore, in order to improve the quality of home-visit care providers, a control mechanism should be established to regulate the size of providers and to convert the private-owned facilities into corporate entities. Since the evaluation system has a positive effect on the improvement of the service quality, it is necessary to utilize the evaluation result in the quality management of the institution.
